Local and national media are welcome to request attending The Electing Convention to be held on June 16, 2007. We ask that everyone please submit a completed request form via one of three ways described below:

OPTION 1:

Open, then print out the form provided below. Fill it out, and send via United State postal mail to:

OPTION 3:

While viewing the form provided below within your web browser (using built-in PDF viewer), either

• Fill all the form fields while viewing the form from within your web browser, then save your updated form to your computer.
  
  
• Save the unedited form to your computer. Once saved, open the the file via the free Acrobat Reader program. Fill in all the form fields, then save your updated form back to your computer.
